With the current dynamic changes in the healthcare landscape in our region, there are numerous opportunities for pharmacists to work with the medical team on enhancing medication use, engage with patients, reduce safety risk and optimize patient outcomes by providing services such as medication management, preventive care, reconciliation, counselling and patient education.
